Registration Agreement

Please write your full name and today's date to indicate you have read and agree to the following policy: I understand I am entering into a tuition contract and I am liable to pay a $15 registration fee for the first dancer, and an additional $10 for each additional family member per family that is registered. I understand that I have chosen auto payment option, and will be liable to the terms listed regarding the tuition payment. I agree to pay tuition on the 5th of each month based on a 9 month payment schedule. I understand the first transaction includes registration, first and last payments.

Tuition Payment Agreement

Please write your full name and today's date to indicate you have read and agree to the following policy: I understand by registering for classes at Gotta Dance Studio By Miss Amy, I have entered into a contract from today's date through the remainder of the session. Tuition payments received later than the end of the business day on the 5th of each month will automatically be charged a $5 late fee on their account. For accounts that are more than one month overdue, an additional charge of 10% of the remaining balance will be accrued. The account balance must be paid before the dancer can resume classes.

Class Withdrawl Agreement

Please write your full name and today's date to indicate you have read and agree to the following policy: If I choose to discontinue classes before the end of the session, I understand that I must notify Gotta Dance Studio in writing to gottadancealbion@gmail.com or amysidari29@gmail.com 5 business days prior to the tuition due date. I understand that I am responsible for paying monthly tuition if the withdrawal process was not completed according to this policy.

Missed Class Agreement

Please write your full name and today's date to indicate you have read and agree to the following policy: I understand that if I or my dancer is unable to attend their regularly scheduled class for 3 weeks or more, Gotta Dance Studio will set up a make up class. If there is an injury we will accommodate the student with seating arrangements to engage in the class and take notes. I understand no refunds will be given for any unused make up classes.
